What To Expect
What to bring for your appointment:
- Insurance Card
- Physician referral forms if required by insurance
- A list of current prescription or over-the-counter medication you are currently taking, including dose and frequency
- Pertinent information about your medical and surgical history. The medical offices where your records are kept may require a medical records request form.
- Any recent x-rays or appropriate records you may have.
