What To Expect

What to bring for your appointment:

  • Insurance Card
  • Physician referral forms if required by insurance
  • A list of current prescription or over-the-counter medication you are currently taking, including dose and frequency
  • Pertinent information about your medical and surgical history. The medical offices where your records are kept may require a medical records request form. 
  • Any recent x-rays or appropriate records you may have.
